In Google Forms, open the form you want to share.

Make sure respondents don't hit a sign-in wall
If your Google Form QR code goes to a Google sign-in screen, two settings are usually the cause. In the form, click the gear icon (Settings):
- Turn off "Restrict to users in [your organization]".
- Turn off "Collect email addresses" (or set it to "Optional" / "Verified" only if you really need it).
- Turn off "Limit to 1 response" if you don't need it - this also requires sign-in.
Generate a fresh QR code from the same form link after changing settings - the link doesn't change, but the sign-in prompt is gone.
